Anno Domini 1562, die vero XII februarii, facta convocatione omnium doctorum ac magistrorum in hypocausto communi iurisperitorum ad audiendam petitionem super admissione ad responsionem pro loco post presentationem dominorum proconsulis ac consulum Cracoviensium prefati magistri Sieprtii, Rome per Collegium medicorum in doctorem in medicinis promoti, ubi visis primum literis sue promotionis, admissus est ad responsionem publicam pro loco, salva tamen omnium primum literarum verificatione et facultatis promovendi archimedici et Collegii medicorum. Que quidem verificatio est facta per legatum pontificis Romani tunc Cracovie presentis {{3}}, eiusque verificationis specialis actus per notarium in librum actorum est insertus sub eiusdem domini doctoris Turobini rectoratu {{4}}. Deinde et dies responsioni, per eundem rectorem, adhibitis sibi venerabilibus ac egregiis viris dominis: Nicolao a Szadek, Sigismundo a Stezicza et Alberto de Posnania {{5}}, collega in medica facultate, die ultima aprilis est prefixa. Conclusio autem facta est super predicta admissione, presentibus venerabilibus ac egregiis dominis: Nicolao a Szadek, theologie, Sigismundo a Stezicza, iuris, Adamo Matla ac aliis facultatum doctoribus ac magistris.
